Terms of Service must be accepted when establishing an account in the Client Portal or when accessing the Portal after a Terms of Service update. Clients who do not use the Portal may submit the paper version instead.

The Veterinary Release form allows the service provider to obtain care from a veterinarian if your pet becomes injured or ill during his or her stay. A signed copy of this form is required from all clients. 

This form supplies client contact and emergency contact information to the service provider.  This information may be entered directly into the Client Portal. Clients who do not use the Portal may submit the paper version instead.
